​New SCM Feature: Outpatient Care Coordination Banner


The Division of Community and Population Health has recently introduced the Outpatient Care Coordination Banner in Allscripts SCM at the Allen, MSCH, and Columbia campuses. This new function quickly highlights the name and contact information of community care coordination and non-primary care team actively working with a patient. This will include ambulatory care management, social work, psychiatry, Center for Community Health Navigation Community Health Workers, Behavioral Health Crisis Stabilization services, and Columbia's complex care pediatrics program. The Banner was created to improve care team visibility of roles involved with our patients; the readily-available information will enable Emergency Department, Inpatient services, and Ambulatory Care Network providers to directly contact other care team members to improve collaboration across settings.

 

To access the Outpatient Care Coordination Banner, simply hover over the words "Care Coord" underneath the 'PCP" field to see Banner information revealed: 


Outpatient and community-based care team members may have information that will help you gain a more holistic view of the patient during your care planning process. They may have awareness of the services (medical, social, behavioral) involved in a patient's care.

 Starting in March 2018, those care team members identified to the Care Coordination banner will start to receive SHM alerts when their patients visit the Allen, Columbia, and MSCH hospital emergency departments and inpatient units.
